This case report is unusual in the severity of the respiratory disease, requiring referral for consideration of lung transplantation with further implications in the clinical management of severe UC. We have reviewed the literature concerning bronchopulmonary manifestations of IBD and discuss the difficulties in distinguishing between a primary manifestation associated with IBD and a side-effect of drug therapy.
